Setup checklist: separate discussion from agreement
Meeting notes often mix ideas, assumptions and commitments. Before closing the record, identify what was actually decided, what remains uncertain and who will do what next. A positive conversation is not the same as approval. Keep the narrative in a dated note and create separate owned actions for valid commitments. Use a three-part close
Write “decided,” “open” and “next” as short sections. Under decided, include only supported agreements. Under open, list questions that affect the next step. Under next, name an action owner and timing. In a fictional scope meeting, “Alex will review the revision” differs from “Alex approved the revision.” Preserve that distinction even when the team wants the opportunity to move forward. Honest uncertainty improves the next conversation and reduces avoidable rework.
Check the action against the relationship
Before creating a task, inspect whether an equivalent commitment already exists. Update the current action or add a genuinely new one rather than producing a parallel reminder after every meeting. Link the relevant opportunity or project where supported. If the meeting changed delivery scope, record the decision in the delivery context too, without rewriting the original agreement as if the change had always been included. Keep the source of the change understandable.
Make the handoff test routine
Ask whether someone absent from the meeting could now act correctly from the record. If not, add the missing evidence or explain the unresolved point. Do not compensate with a long transcript that hides the decision. A concise summary plus reliable underlying context is often more useful. Continue to qualification evidence when the commercial stage is uncertain, or the task-versus-note guide when the problem is distinguishing history from current responsibility.
